Weight plays a significant role in fertility, including the success of in vitro fertilization (IVF). A recent large-scale study sought to determine how a patient’s body mass index (BMI)—a measure of weight in relation to height—affects the chances of IVF success. The results provide important insights for intended parents looking to optimize their chances of a successful pregnancy.

Understanding BMI and Its Categories

BMI is used to assess a person’s body weight relative to their height, with the following categories:

  • Underweight: BMI below 18.5
  • Normal weight: BMI between 18.5 and 24.9
  • Overweight: BMI between 25 and 29.9
  • Obese: BMI over 30 (with subcategories for BMIs over 35 and 40)

Studies show that both low and high BMI can impact natural conception and IVF success, with BMI extremes associated with lower pregnancy rates and a higher risk of complications.

The Impact of Low and High BMI on IVF Success

Researchers conducted an extensive study on the outcomes of over 77,000 IVF cycles in the United States. They examined cycles in which the embryos had undergone chromosomal testing (PGT-A) to ensure high embryo quality. Here’s what they found:

Optimal BMI for IVF Success

The highest pregnancy and live birth rates were observed in women with a BMI between 23 and 25. This suggests that maintaining a BMI within the normal range may improve chances of IVF success.

Reduced Success Rates for Low BMI

For women with a BMI below 20:

  • A BMI between 18.5 and 20 resulted in a 7% decrease in live birth rates.
  • A BMI below 18.5 showed an 11% decrease in live birth rates.

Women with very low BMI were more likely to experience lower implantation rates, meaning embryos were less likely to successfully implant in the uterus.

Reduced Success Rates for High BMI

For women with BMI above 25:

  • A BMI between 25 and 30 led to a 4% decrease in live birth rates.
  • A BMI between 30 and 35 saw a 9% decrease.
  • A BMI between 35 and 40 resulted in a 15% decrease.
  • A BMI over 40 showed a substantial 30% decrease in live birth rates.

For both underweight and obese women, there was also an increased likelihood of pregnancy loss between a positive pregnancy test and the first ultrasound.

Why Does Weight Affect IVF Success?

The challenges with low and high BMI may relate to how these weight extremes impact hormone balance, egg quality, and overall reproductive health:

  • Underweight women may have insufficient body fat, affecting hormone levels needed for a successful pregnancy.
  • Overweight and obese women often experience higher levels of inflammation and hormonal imbalances, potentially impacting egg quality, embryo development, and the endometrial environment, which are critical for embryo implantation.

Can Weight Adjustment Improve IVF Success?

While these findings suggest that a balanced BMI can improve IVF outcomes, studies have not yet conclusively shown that weight loss or gain directly improves success rates for women undergoing IVF. Research in this area is challenging due to individual differences in metabolism, hormone levels, and lifestyle factors.

For those planning for IVF, maintaining a balanced BMI can help optimize the likelihood of success.
